- What is retinal detachment? Why does it happen? What are the key factors that determine the prognosis in patients with retinal detachment and their potential response to treatment?
- So, retinal detachment is when there is a hole or a tear in the retina, which basically means that fluid can get behind from the front of the eye in the vitreous chamber through the hole into the retina, so that the part of the retina that senses light, comes away from the rest of the retina, and that basically is seen by the patient as almost like a curtain falling, coming down over their vision. So, it's a very recognizable thing because the patient suddenly loses their vision, and, of course, they invariably want to do something about it. One of the things that is very difficult to predict is who is going to do well with the surgery. We do know that when the retina detaches and it involves the macula, so the macula is, if you think of a pinhole camera, all the rays of the camera are focused to one central point. The same thing goes on in the eye, the macula is where you have your sharpest definition of the image. Now, if you have a retinal detachment, which involves the macula, the prognosis is a bit less good because of the fact that, that whole very complex structure has come away from its support structure and therefore you don't know if that patient necessarily will fully recover their vision. There is some evidence suggests that actually that any separation at that point means that your patient will never go back to the best possible vision. But what you try and do is, because usually, the tear of the retina is coming from a particular side or in the periphery of the retina, usually you hope that the patient presents very quickly to their eye specialist before their macula becomes detached and that increases the awareness and the prognosis for improvement in the vision. - And for patients with retinal detachment sometimes they have a repeat retinal detachment. - Yes. - How often does retinal detachment recur after the treatment? Because sometimes it's a recurring problem. - So, it shouldn't be but it sometimes can be, I mean, the people who are at risk are those people who are very short-sighted, you've probably are aware that you if are very short-sighted that just for simplicity, your eye geometry changes -- Yes... Stretching of the retina occurs, the retina being thinner means that [short-sighted people] are more susceptible and they do possibly have more repeated risk of retinal detachments. However, a lot of the time nowadays the actual procedure involves intraocular surgery so there are some ways of doing this retinal detachment repair, where you put some sort of buckle on the outside of the eye to get that bit of the retina that's come away to stick. But often, you would do a vitrectomy, which is taking the jelly out of the eye, and at the time, what you would do is you obviously have a very good look around to check that no potential holes or there are no little holes that could have been missed, to ensure that those don't cause a problem. And that's one way of ensuring that you don't have a repeat retinal detachment.
