- If someone has to have surgery to replace heart valve, how to decide between tissue and mechanical heart valve? Especially for elderly people? As you point out, there are only two kinds of replacement heart valves. The mechanical heart valve, which is made of artificial materials. And there is a tissue valve, which is made of cow tissue or pig tissue. Current recommendation is that everyone over 60 or 65 years of age should have a tissue heart valve as replacement valve. And everybody younger than 60 or 65 should have a mechanical valve. The problem with mechanical heart valve is that the patient has to take blood thinning medication every day for the rest of patient's life. Patients also have to do blood tests frequently. A lot of people do not like to do that or they have a disease when anti-coagulation with Coumadin or warfarin is prohibited. So in the United States, over 70% of patients we treat, regardless of their age, choose an animal tissue heart valve. So that they do not have to take blood-thinning medication. Patients have to understand that they will require a repeat operation in 12 to 15 years time to replace animal tissue valve to a new valve. But elderly patients often receive animal tissue valve replacement, because elderly patients do not tolerate anti-coagulation well, or have other medical problems that may interfere with anticoagulation drugs. So in the United States, the choice of mechanical or tissue heart valve depends on a free choice of the patient, with knowledge of advantages and disadvantages of both [heart valve types]. But a lot of younger people, who want to ski or play hockey, which they cannot do if they take blood-thinning medications, choose animal tissue replacement heart valve. They know that they will have to have repeat heart surgery sooner than they would if they had a mechanical heart valve. But a lot of decisions depend on their lifestyle. Risks of repeat heart surgery in 12 to 15 years for a patient with animal tissue heart valve are similar to the risks of the first operation? In our hands - yes, in some hands - no. But in our hands the results are very good. But the patient should not wait too long - when the valve becomes stenotic or calcified, they should prepare for repeat surgery, rather than wait until they get really sick.
